AU Health has the only kidney transplant center in Georgia outside of Atlanta. Patients can complete pre-transplant evaluation testing and post-transplant follow-up care at our main location and several satellite clinics throughout Georgia and South Carolina.

Referrals may be submitted by fax to 706-721-1394 or via Transplant Referral Exchange (TREX). Please include the following documents and information necessary for timely, accurate processing of patient referrals:

1. Transplant Referral Form
2. Copy of the front and back of the patient’s insurance card(s)
3. Form 2728, if patient is on dialysis

Our staff will keep you informed of each patient’s progress throughout the transplant evaluation process. We also mail patient status reports to referring physician and dialysis facilities on a quarterly basis.

Who should be referred?

Patients can be referred to AU Health's kidney transplant program if they: 

• Are progressing to end-stage renal disease.
• Want to learn more about kidney and/or pancreas transplants.
• Have a body mass index (BMI) of 40 or lower. Calculate your BMI.
• Have adequate psychosocial support and reliable transportation.
• Are free from substance dependence (alcohol and illicit drugs).
